ANIMAL FACILITY REGISTRATION (ANIMAL WELFARE REGISTRATION)
The Animal Welfare Act (Republic Act 8485) is a law designed to protect and promote the welfare of all animals in the Philippines by regulating the establishment and operation of facilities used for breeding, maintaining, keeping, treating, or training animals, whether as objects of trade or as household pets. The division is responsible for the registration and licensing of animal facilities to ensure compliance with RA 8485 by those engaged in the business.

- Animal Facility Application Form with 1×1 Picture of Applicant
- Proof of Registration/Creation as an Establishment
- Valid ID of the Officer-in-Charge, Owner, Manager, or Veterinarian
- Valid PRC ID of the Facility Veterinarian
- Valid Professional Tax Receipt (PTR) of the Facility Veterinarian
- Notarized Employment Contract/Memorandum of Agreement/Appointment Order of the Facility Veterinarian
- Notarized Valid Contract of Lease of the Space/Building occupied
- Location Map/Sketch of the Establishment
- Certificate of Attendance of Owner/Facility Veterinarian from Animal Welfare Seminar
- Certificate of Non-Coverage (CNC)/Environmental Clearance Certificate (ECC)
- Picture of the Facility & its Equipment
Other Facility-Specific Documentary Requirements
- Valid S2 License of the Veterinary Facility, Shelter, Pound or Laboratory Animal Facility (Except Laboratory Animal Production Facility)
- Animal Care and Use Program (ACUP) and Institutional Animal Care and Use
- Certificate of Wildlife Registration (CWR)/ Wildlife Farm Permit (WFP) for Captive Wildlife Facility

- Copy of Business/Mayor’s Permit

- Upon renewal, a surcharge of 50% of the amount due shall be levied on every expired registration certificate.
