REGISTRATION AND LICENSING OF FEEDS ESTABLISHMENTS

The Registration and Licensing (RL) Section registers and licenses feed establishments to ensure that they operate lawfully and adhere to rules and regulations specifically related to the manufacture, importation, exportation, labeling, advertising, distribution, and sale of livestock, poultry, aquaculture, and specialty feeds. This includes conducting preliminary investigations into reported malpractice or fraudulent sales of unlabeled, adulterated, or substitute products. Additionally, it provides for the evaluation of both commercial and non-commercial feed establishments.

REQUIREMENTS TO BE BROUGHT/ SUBMITTED

A. COMMERCIAL MIXED FEED (Commercial Feed Trader/Toll)/ Feed Ingredient Manufacturers)

  1. For business organization
    • Sole Proprietorship (DTI Registration)
    • Corporations (SEC Registration & Articles of Incorporation & ByLaws)
  2. Photocopy of Environmental Compliance Certificate (ECC) from EMB/Permit to Operate (PTO) issued by the DENR Provincial Office (PENRO)
  3. Business /Mayor’s Permit
  4. Plant Lay-out
  5. Copy of Memorandum of Agreement (MOA) between parties for Toll Feed Manufacturer or Commercial Feed Trader
  6. Joint Affidavit of Undertaking of Owner with Licensed Agriculturist (Animal Nutritionist) or Veterinarian, and Chemist (RF FVDV30) with photocopy of PRC ID
  7. Copy of valid BAI-Laboratory Recognition Certificate & MOA between Parties for Manufacturers contracting the services of an independent Quality Control Laboratory
  8. Site Inspection of office, plant & warehouse & validation of submitted documents
  9. Sketch or Location Map of the Establishment or Facility
