About the Service
It is the purpose of this Act to protect and promote the welfare of all animals in the Philippines by supervising and regulating the establishment and operations of all facilities utilized for breeding, maintaining, keeping, treating or training of all animals either as objects of trade or as household pets. For purposes of this Act, pet animal shall include birds.

General Requirements
- Xerox copy of Mayor’s Permit for the current year.
- Xerox copy of business name with BDT/SEC (if corporate) and Articles of Inc.
- ID picture of the Owner/General Manager and Veterinarian
- Xerox copy of veterinarian registration / PRC ID and PTR
- Notarized valid contract of leases of the space / building occupied (if applicant does not own)
- Location plan
- Certificate that the owner/veterinarian has attended a sponsored-accredited seminar on Animal Welfare Act
- Notarized financial statement
- Environmental Clearance Certificate (ECC) from the Department of Environmental and Natural Resources (DENR)
